两者意思大致相同，不同的是语气：一个是真实的、另一个是虚拟的。虚拟语气有多种用法，这里只是表示委婉的语气。有教养的人在请求别人帮助的时候，都会使用委婉的说法。类似的用法如：Could you tell me your phone number?

第一句话就是普通的请求，意思是：如果你能帮我，我会非常感激。

the "if you can help me" one means that if you help me, i'll definitely be grateful.

the "if you could help me" one means that maybe you'd help me but the fact's i have no fking idea if your gonna help me and i can't guarantee that i'd give a fuck about ya after you helping me.

and as and canadian i tell you that we say it in both ways which means....people ain't think about such nonsense stuff when they're talking, alright?
